What does the program look like?
Orthopedic rehabilitation will be unique for every individual based on your health, skills, goals, etc. Your healthcare team builds a specific treatment plan for you and adapts it as you recover.
First, the severity of your sickness or injury dictates whether you should pursue rehabilitation as an outpatient or as part of a post-acute inpatient program. Outpatient rehab is usually indicated for mild injuries, whereas inpatient rehab is generally recommended for more severe conditions.
An appropriate transfer of care is required at the end of orthopedic rehabilitation. Your healthcare team will give you the tools and services you need to make the most seamless transition back to your home or to a long-term care facility. Making safety adjustments to your house, scheduling caretaking and transportation obligations, follow-up needs, and self-management approaches for your condition are all examples.
